Too many people are dying alone and in pain.
Terminally ill people are spending their final moments in overcrowded A&Es, ambulances and hospital corridors because they can't get palliative care at home when they need it. Families feel abandoned and have nowhere to turn, because end of life care services and staff are stretched to the absolute limit.
Wes Streeting, the Secretary of State for Health and Social Care, knows that end of life care is in crisis. He's said it himself. But the UK Government aren't taking any action to fix it, and dying people are paying the price.
Right now, his Department are working on a 10 Year Plan for NHS England and deciding what services to invest in. We need to make sure end of life care is at the heart of those plans - because an NHS that's fit for the future must also be fit for dying people.
Let's remind him that he has the power to transform how people are looked after at the end of their lives.
